Ohio State offers a nice web guide with examples.

Be careful!  The Chicago Manual has two distinct formats.  This course, and most humanities/social sciences publications, use the Notes-Bibliography (NB) system.  The sciences use the Author-Date (AD) system. 

Images deserve credit, too!
